I cant get it underneath the revolutionary slider and there is no info on the boxes.. Please help

If you can provide us WP Logins to your website with URL then we can take a closer look at it. Make sure you set the post as private reply so that only admin can see your wp-logins.

Oh and is it possible to take off department section on appointment template.. I don’t need it and don’t need the doctor section that pops up with it.

Sorry removing the doctors section isn’t possible but department is possible which you need to select only a specific category for that department in the theme options panel. If you wish to use this for the custom development then please use contact form 7 plugin where you can create custom form with customized fields.

Regarding underneath the revolutionary slider and there is no info on the boxes question you need to set the template as fullwidth

MAQX thanks for fixing the appointment tab on the home page template.

I still need the info inside the appointment box which says Name, E-mail, Date, and Phone Number. If you see on my home page it is still empty. I still using the same log in information as the one I gave you. Or maybe you can write some instructions on how I can do it myself because I see a lot of people with the same theme asking the same question.

My Friend I apologize maybe we where looking in different explorers

In Internet Explorer – It does work there

Google Chrome – Unfortunately Name/Email/Date/Phone on appointment in Home page does not work they are empty

I switched the topic on top from resolved to In Progress.
