Is there a way to make these fields required: Last Name, Date of Birth, and Gender on the appointment form? Also is there a way to add fields to the form? We are also having an issue of appointments being duplicated, some get duplicated three times. Please let me know.

Hi NycheDesigns,

Fields required for Last Name, Date of Birth, and Gender is possible. Adding fields to the form feature is not available.

I would like this option as well as having phone number required.

Adding fields to the form will be custom work so it will charge extra dollars.
