1. First you need to create a custom sidebar then move to artist page select the layout and select the custom sidebar that you have created after that please visit the widget area assign the widget that you want to display in the side bar.

2. You need to install the contact form 7 and create a contact form then copy the shortcode paste it in a contact page.
