Hi,
I just purchased your theme via theme forest and I want to have exactly the same page as it is shown on http://themeforest.net/item/lifecare-responsive-medical-wordpress-theme/full_screen_preview/7813491
Including appointment form on front page and exactly the same revolution slider, I only want to change text, pictures and colors on the site, evertthing else I want to be the same.
Create a front page and go to settings– reading and make it as fornt page as shown in screenshot below.
