    Is it possible to change the fields on the appointment request? for example, I’d like to remove the date field, add a field for type of request (Residential, Commercial, Emergency) and also would like the last name on the form as well.

    Hopefully this is easy to do inside the template without major custom coding. An option to select which fields to display on the request would be great.

    Sorry, adding the additional fields in the quick appointment is not possible as it is a shortcode. If you wish to make a change then you can do that in the appointment template but not in quick appointment form.

    The appointment form directory path is ..\wp-content\themes\anona\pest-control\appointment before changing the files take a backup of your theme and do it in the child theme so will not lose your data.
