If you can provide us WP Logins to your website with URL then we can take a closer look at it. Make sure you set the post as private reply so that only admin can see your wp-logins.

seems, that a plugin is missing?

Yes, you have to install contact form 7 plugin in your theme.And from there you have to create new contact form shortcode. After creating, you can use that shortcode whereever you want.

This is the plugin you have to install, https://wordpress.org/plugins/contact-form-7/
