Hello, After a user clicks the Make Appointment button, a Confirm Appointment page does not appear. Help, please. Thank you.

There is no appointments confirmation page, it just displays the Alert style box message saying that your appointment was booked etc. if you are unable to see a message on your server make a test mail function and see if your server is not using SMTP for sending emails

Please provide us the URL to take a closer look at it.

I am not even seeing an Alert box saying that the appointment was booked. How do I create an appointments confirmation page?

If you can provide us WP Logins to your website with URL then we can take a closer look at it. Make sure you set the post as private reply so that only admin can see your wp-logins.
Note: Before providing the wp-logins to any support staff, kindly please make sure you take a backup of everything including wordpress content, theme options, widgets etc.

As I have seen you are using an older version this theme #3.1.0, please update the latest version to #4.1.0

If you check in the documentation we have a section called Updating theme
which explains you on how to update the theme in 2 methods please follow that and do take a backup before you take any step.

Try contacting to ThemeForest and I think they will be able to help you out with the downloads list disappearance.

For your help in updating in meanwhile you get that support query from envato you can go to this link and follow as said. If you still have any doubts please feel free to ask.
