Hi,

I like the look of the appointment box, but I cant really use the internal wordpress App to setup appointments. is there a way that the customer can just fill out the information, and it sends it to our e-mail address so we can follow up with that customer?

Set your default appointment to confirm appointment. So every customer will get a confirmed email as well as admin too. But for this you need to setup admin email in your theme options.
