Can you tell me how to resolve the issue with the appointment call out form displaying the date field as read only? Nobody can enter any dates.

Follow this path plugins/anona-core/anona-shortcodes/shortcode/appointmentcallout.php file and find for this readonly="readyonly" and remove it.
