We have 1 doctor. When we add him to the site and delete the rest, no names show up in our appointments form. See: https://fidelitytelemed.com/appointments/.

If you can provide us WP Logins to your website with URL then we can take a closer look at it. Make sure you set the post as a private reply so that only admin can see your wp-logins.
Note: Before providing the wp-login to any support staff, kindly please make sure you take a backup of everything including WordPress content, theme options, widgets etc.

The Appointment dropdown list not showing because in the Department you didn’t select the Doctor Specialty and Doctor Name. Now, We had set up the appointment dropdown list.
