Looking at the Contact-Us page in the theme, it looks like the shortcode “contact-form-7” doesn’t work? I don’t see an option for that shortcode, but I see that it works on your theme’s demo page. My page is below.

You need to install contact form 7 plugin and then create form place the shortcode it will work fine.
